As many of you may know, Dr. Michael Lyons passed away on December 8, 2009 from a sudden stroke. He is incredibly missed by his family, his friends and his professional colleagues. We know that he enjoyed the opportunity to assist each and every one of you, his patients. Dr. Lyons was a solo practitioner and as such, does not have a partner who we can entrust his practice to. So, unfortunately, we will be closing his office on April 1, 2010. We urge you to retrieve your medical records prior to March 30th, 2010.

4. Please mail the consent form along with a self-addressed stamped envelope to the address below. We suggest sending a large manila envelope as many of the records are quite voluminous.
